Therapeutic Profile

• Schedule II controlled substance
• Onset/Duration:

• Immediate-Release (IR): 15-30min onset, 4-6hr duration

• Extended-Release (ER): 1-2hr onset, 12hr duration
  • METABOLISM: Hepatic (CYP3A4/2D6)

Clinical Indications

✔ Acute post-surgical pain (IR only)
✔ Breakthrough cancer pain (with ER baseline)
✔ Severe injury trauma (short-term IR use)
✔ Chronic pain (ER only for opioid-tolerant patients)

Dosing Guidelines

For OPIOID-NAÏVE Patients (IR only):

• Start with 5mg q4-6h PRN

• Titrate cautiously → max 10mg/dose

For OPIOID-TOLERANT Patients:

• ER: 10mg q12h (must confirm ≥20mg MEDD baseline)

• IR: 10mg q4-6h PRN breakthrough pain

Critical Administration Notes:

• ER tablets: Swallow whole – never crush/chew

• IR tablets: May be split for dose adjustment

• Food effect: High-fat meals ↑ ER absorption 25%

Abuse-Deterrent Formulations

• ER versions: OxyContin OP, Xtampza ER

• IR versions: Oxaydo (nasal abuse-resistant)
