• Onset/Duration:

Immediate-Release (IR): 15-30min onset, 4-6hr duration

Extended-Release (ER): 1-2hr onset, 12hr duration
• METABOLISM: Hepatic (CYP3A4/2D6)
Clinical Indications
✔ Acute post-surgical pain (IR only)
✔ Breakthrough cancer pain (with ER baseline)
✔ Severe injury trauma (short-term IR use)
✔ Chronic pain (ER only for opioid-tolerant patients)
Dosing Guidelines
For OPIOID-NAÏVE Patients (IR only):

Start with 5mg q4-6h PRN

Titrate cautiously → max 10mg/dose
For OPIOID-TOLERANT Patients:

ER: 10mg q12h (must confirm ≥20mg MEDD baseline)

IR: 10mg q4-6h PRN breakthrough pain
Critical Administration Notes:

ER tablets: Swallow whole – never crush/chew

IR tablets: May be split for dose adjustment

Food effect: High-fat meals ↑ ER absorption 25%
